How to Choose an AI Receptionist or Automation Tool for a Med Spa

What 21 AI receptionist and automation vendors publish about price, terms, HIPAA and proof, and the questions to ask before you choose one.

An AI receptionist answers your phone, your texts and your website chat, books appointments and follows up on leads without adding a person at the front desk. This guide covers what the 21 vendors in our AI and automation category publish for a med spa buyer. We read every vendor's website on October 1, 2026 and recorded what each one states about price, terms, integrations, privacy and proof. Every figure links to a vendor profile, and each profile links to the page the figure came from. We have not used these products, so this is a guide to what is published, not a recommendation.

What does an AI receptionist do for a med spa?

Most of the vendors here do one of four jobs. Answer the phone: AI voice agents such as AgentZap, Clara AI and MedspAI pick up, answer questions, then book the caller or transfer the call. Answer chat and DMs: tools such as Hyperleap AI and SignalCore put an agent on your website, WhatsApp, Instagram or Messenger. Humans plus AI: Abby Connect and Smith.ai pair live receptionists with AI answering, and My Salon Desk sells hybrid plans where the AI can transfer a call to a human. Plumbing and tracking: Keragon automates workflows between healthcare systems, Liine does call tracking and marketing attribution, Hello Patient and Podium sell broader patient communication, and Krazimo is a custom engineering firm that builds AI receptionists for med spas.

How much does an AI receptionist cost per month?

14 of the 21 publish a starting price, and 13 of those publish a monthly starting price in dollars:

The other 7 do not publish a price: Hello Patient, Krazimo, Liine, MedspAI, Podium, Recura and Relay AI (chatbotservices.net). Four of the seven say how they charge: Hello Patient and Liine describe usage-based pricing, quoted per practice, Podium says quote only, and MedspAI publishes plan names and included call volumes with prices on request.

The spread matters less than the meter. BookingBee.ai charges per outlet, Zellyfi per website, and My Salon Desk charges med spa and medical practices more than salons on the same plan: $495/mo against $445/mo at Start-Up, $1,150 against $950 at Growth, and $1,850 against $1,550 at Advanced. Work out your monthly call volume first, then price each shortlisted vendor at that volume.

What will you pay beyond the monthly plan?

Setup and onboarding. 10 of the 21 state a setup or onboarding fee policy. Eight state there is none: Abby Connect, BookingBee.ai, Hyperleap AI, Liine, MedReception AI, MedspAI, Zellyfi on its AI assistant plans and AI Receptionist Med Spa (Wellgrow). Two charge: AgentZap lists a $499 one-time training and setup fee, which its Professional plan waives on a 6-month term, and My Salon Desk charges an onboarding fee equal to one month of the plan price.

Metered overage. Abby Connect bills overage at the plan rate, with AI answering drawing minutes at half the human rate. AgentZap's overage runs from $0.70 to $0.85 per minute, MedReception AI charges $1.25 per extra minute on its Essential plan, and Keragon bills runs beyond the allowance at $0.02 to $0.15 each. Smith.ai's free tier costs $3.00 per call past 25 calls a month, with appointment booking at $1.50 and recording and transcription at $0.25 as add-ons.

Refunds. My Salon Desk gives no refunds for any plan or add-on and MedReception AI gives none for unused minutes, while Smith.ai and AgentZap offer a 30-day money-back guarantee to new clients.

How long are you locked in?

12 of the 21 state a month-to-month term, including Abby Connect, Clara AI, MedReception AI, Smith.ai and AI Receptionist Med Spa (Wellgrow).

Where a commitment appears, it sits in a specific place: AgentZap's managed option runs on a 6-month term while its standard plans have no contract, MedspAI offers month-to-month or a 6-month commitment, My Salon Desk says some plans may require a 3 month contract after the first month, and Liine sets the initial term in the order form, renewing automatically, so the length is set in the order form you see during the sale.

8 of the 21 do not state a minimum term. For Hello Patient, Keragon, Krazimo, Podium, Recura and Relay AI (chatbotservices.net) there is no published term and no published cancellation policy, so the exit terms arrive with the contract. Where cancellation is published, read the notice period: Abby Connect asks for at least 30 days notice before a renewal date.

Will it book into your scheduling system?

An answering service that cannot reach your calendar produces messages someone types in afterwards. 10 of the 21 name a salon, spa or med spa scheduling system in their published integrations. MedspAI lists compatibility with med spa EMRs including Aesthetic Record, AestheticsPro, Boulevard, Mangomint, PatientNow, Symplast and Zenoti, and Clara AI, My Salon Desk, SignalCore and AI Receptionist Med Spa (Wellgrow) name Boulevard, Vagaro, Zenoti or Mangomint. AgentZap lists Mindbody, Booker and Zenoti, BookingBee.ai lists Meevo, Mindbody, Phorest and Square, Relay AI (chatbotservices.net) books into Vagaro, Boulevard, Mindbody and Jane, Recura integrates with PatientNow, and Liine connects to Zenoti. The rest publish generic connections, healthcare EHR integrations or no list at all. If your calendar is not named, ask how a booked appointment reaches it.

What about HIPAA and patient privacy?

If your calls include patient names and treatments, ask how a vendor handles that before you forward a single call.

11 of the 21 state a HIPAA or BAA position. Keragon says a BAA is included on all plans, Hello Patient signs a BAA with every practice, MedReception AI signs one during implementation, and Liine, AgentZap, Clara AI and AI Receptionist Med Spa (Wellgrow) state BAA positions of their own. Ruby offers a HIPAA-compliant option. Podium and Recura say they comply with HIPAA, with no BAA statement on the pages we read, and Hyperleap AI's med spa page describes HIPAA-aware guardrails.

One vendor draws the line in public: Smith.ai states that it is not HIPAA-compliant and cannot handle protected health information. For the remaining 9 we found no HIPAA compliance or BAA statement for the service, and silence is not the same as compliance. Ask for the BAA in writing and ask which parts of the service it covers.

What proof do vendors publish?

4 of the 21 publish a case study result with a number in it, and only one is from a med spa: Clara AI reports that after-hours booking at Elevate Aesthetics & Wellness went from 0% to 94%, with $9,400 in monthly revenue recovered, as stated by the vendor. The others sit outside the category: BookingBee.ai quotes a salon owner on 300+ after-hours calls captured and $6,000 generated, Hello Patient reports an urgent care client cutting missed calls from 26% to zero in 30 days, and Liine credits Eleanor Health with 20% more bookings in 90 days. Named clients follow the same pattern: Recura names one med spa, Lone Tree Aesthetics Medical Spa, while BookingBee.ai names salons, Liine names dental and eye care groups, and Hello Patient names urgent care, orthopedic and mental health clients.

Only 2 of the 21 are built for med spas alone: AI Receptionist Med Spa (Wellgrow) and MedspAI. A med spa page elsewhere can be a landing page rather than a list of work done for med spas, so ask for a reference from a practice that looks like yours.

Who owns the data and the phone number when you leave?

Ownership is not something we recorded for most vendors in this category, so it is a question to ask. MedspAI says it handles number porting during onboarding, but published material does not state who owns the number when you leave. Pin down in writing: who owns the recordings and transcripts, whether the trained agent setup can be exported, how your number is ported out, and where the lead data lives after cancellation.

What should you ask on a sales call?

  1. What is the total monthly cost at my call volume, including the overage rate once the included minutes run out?
  2. Is there a setup, onboarding or training fee, and is it refundable?
  3. How long is the contract, how does it renew, how much notice does cancellation need, and which payments are refundable?
  4. Will you sign a BAA, and which parts of the service does it cover?
  5. Does the agent book straight into my scheduling system, or does it pass bookings to staff to enter?
  6. Who owns the call recordings, transcripts, lead data and the phone number if we leave?
  7. Can we trial this on our own lines, and can you connect us with a med spa client of a similar size?
