PatientGain's $99 figure is one app, not a full-service retainer. Its service plans are listed at $899/mo, $1699/mo and $2499/mo (GOLD, PLATINUM, PLATINUM Plus Service), and the same page also says pricing starts at $499/mo.
Omnicore and Sprout Sage Solutions publish starting figures that are not the monthly kind the table lists: Omnicore lists SEO from $1,500 and does not state the billing period; Sprout Sage Solutions starts at $300 one-time for a landing page, with SEO retainers from $800/mo.
These prices are not like for like: Intrepy's $750 covers SEO for a solo physician, More Booked Consults' $2,500 buys one service, CRO or local SEO; both together are $5,000/mo and managed Meta ads are $3,000/mo.
What will you pay beyond the monthly fee?
Setup fees and deposits. 7 of the 26 state a policy. Intellius Medical and RUNNER Agency state none. PatientGain waives it on a 12-month contract and charges one on month-to-month or shorter terms. More Booked Consults charges a $2,500 one-time setup, on managed Meta ads only. Etna Interactive takes a deposit with the signed proposal; Plastic Surgery Studios requires a non-refundable one before work begins, applied to the final invoice. Neither publishes the amount.
Ad spend. Intellius Medical and Stranded say ad spend is paid directly to the platform; Thrive Internet Marketing Agency states it is not in the monthly budget. More Booked Consults requires at least $3,000/mo in Meta ad spend, paid directly to Meta. PatientGain takes an 18% fee on managed Google and Meta ads on its Growth tier. Intellius Medical, Sprout Sage Solutions and Stranded charge flat fees, not a percentage of spend.
Extras. Omnicore lists a Practice Pulse Dashboard add-on at $97/month and extra web pages at $150 each. Growth99's terms include a $35 late fee and a $100 reactivation fee. MyAdvice adds a 3% service fee on credit card payments and a $50 handling fee on checks.
How long will you be locked in?
13 of the 26 state their contract terms.
- Month to month. 8 of the 26 state it: Incredible Marketing, Intellius Medical, Med Spa Marketing, Medspa Market Pro, Omnicore, PatientGain, Sprout Sage Solutions and Thrive Internet Marketing Agency. Sprout Sage Solutions asks for 30 days notice. PatientGain's month-to-month option carries a setup fee. Surgeon's Advisor also states no long-term contract.
- A year. Growth99 requires a 1-year term, renewing automatically for successive 1-year terms. PatientGain's other option is a 12-month contract with no setup fee. Plastic Surgery Studios requires 12 months for any product paid in monthly installments.
- Six months or less. Stranded runs 6 months, then month to month. More Booked Consults runs 6 months for CRO and local SEO and 3 months for managed Meta ads.
- Not stated. The other 13 do not state a term, so ask the length before you sign.
What happens when you want to leave?
9 of the 26 publish cancellation terms:
- Incredible Marketing: cancel any time with 60 days' notice.
- Sprout Sage Solutions: retainers end on 30-day written notice; project deposits are non-refundable once work has started.
- More Booked Consults: month to month after the minimum term, cancel by email with 30 days' notice, no cancellation fee.
- Med Spa Marketing: contracts renew automatically unless you give written non-renewal notice at least 30 days before the term ends.
- MyAdvice: the agreement auto-renews; written notice at least 45 days before renewal, and ending early still owes the rest of the term.
- Growth99: non-renewal notice at least 60 days before the term ends; its terms state all sales are final with no refunds.
- Plastic Surgery Studios: either party may terminate without cause on 30 days' written notice, except monthly-installment services in the first 12 months.
- Intellius Medical states no cancellation penalty. Medspa Market Pro says cancel anytime.

- Studio 3 Marketing: paid ads run from the client's own account, so the client keeps control and ownership.
- Plastic Surgery Studios: after payment in full the client owns the final website, blog and marketing pages; working and source files stay with the agency.
- MyAdvice: clients own text and visual content created by them, the platform's AI tools or MyAdvice; packaging it up on exit is billed hourly.
- More Booked Consults: the practice owns its Meta ad account, pixel and audiences, website, domain, Google Business Profile and analytics, and keeps the landing pages, ads, test results and reports.
- Etna Interactive states clients own all their assets if they leave; Incredible Marketing states clients keep the websites, content and software it creates, including after leaving.

A number without a name is harder to check, so ask for references. 24 of the 26 offer a free demo or audit; Medspa Market Pro is the only one that states a free trial. PatientGain publishes a HIPAA statement for its apps.
